Reports are expected to be well organized, original, accurate, and complete. The written report must include the following information:

1. Introduction and chief complaint
2. Medical and dental histories
3. Diagnosis and treatment plan
4. Description of treatment
5. Minimum of three references to literature pertaining to the treatment prescribed
