Must be an Academy of Comprehensive Esthetics member in good standing during the entire process
Must have attended one of the last two Academy of Comprehensive Esthetics Annual Meetings, which can include the meeting at which the written exam was taken.
Must submit the two required Clinical Cases within one year of passing the Written Exam
Must submit all required information (written report, photos, and radiographs) on three discs to: Dr. James Sarant at 3713 Benson Dr. Ste. 102, Raleigh, North Carolina, 27609.
Written Report
Reports are expected to be well organized, original, accurate, and complete. The written report must include the following information:
Introduction and chief complaint
Medical and dental histories
Diagnosis and treatment plan
Description of treatment
Minimum of three references to literature pertaining to the treatment prescribed
Photographic Documentation
Photographic documentation is to be submitted in the form of digital JPEG images of approximately 1mb in file size per image, utilizing the specific Power Point format provided. The following photographs must be taken for each case type:
Anterior Case
Twenty-eight (14 pre-operative, 14 post-operative) photographs are taken for the anterior case and sequenced exactly as shown in the Academy of Comprehensive Esthetics Digital Photography Guide. Photos must be taken as specified in the table below.

Once the file has been downloaded you will be able to click on the picture icon on each slide in the template to import photos of your case from your computer into the slide. By using the picture icon the photos will automatically be resized to fit the template.
Posterior Case
For the Posterior Case, there are two required views. Each view below is required Pre-Operatively, of the Preparations, and Post-Operatively.
Required Views for Posterior Case:
Right Retracted- in occlusion 1:2 OR Left Retracted- in occlusion 1:2 (depending upon which side is treated)
Maxillary Occlusal 1:2 OR
Mandibular Occlusal 1:2
(depending upon which side is treated)
Photographic Release Form
The Academy of Comprehensive Esthetics photographic release form must be signed by the patient, giving permission to the Academy to use his or her image.
Verification of Authenticity for Each Case
Recent changes in technology, which allow photographic images to be altered, create a potential for fraud, so we have taken precautions to insure that all submitted work is original. We expect candidates to be honorable in submitting their own work. If alterations or tampering of the digital images has occurred, the candidate will be immediately disqualified from Fellowship and will be banned from applying for the credential again. The Candidate’s name will also be given to all other organizations which offer advanced credentials in esthetic and restorative dentistry.
Radiographic Documentation
Pre-operative and post-operative radiographs are required for all cases. An implant must also include pre-operative radiograph(s) prior to implant placement. Radiographs must be of diagnostic quality. Use as many radiographs as needed to show the basis for the comprehensive treatment plan.
Case Evaluation
Each case is judged anonymously by a panel of FELLOWED examiners. None of the examiners will know who has submitted the case AND none of them will know which other examiner is looking at the same case. Therefore there will be NO chance for collusion or nepotism. A majority of "pass" votesare required to pass a case. The candidate may resubmit Clinical Case Reports a maximum of two (2) times. If not successful the candidate must redo the failing case or cases again, with an entirely new case or cases.
When both Clinical Case Reports have received a passing grade, the candidate is eligible to continue to Phase 3, the Oral Presentation.
